Your employees are already using AI tools. ChatGPT, Microsoft Copilot, Google Gemini, Claude — they’re in your inbox, your documents, your customer support workflows, and your development pipelines. Most of them got there without a single security question being asked.
This post introduces the AI Vendor Security Questionnaire for Business — a 40-question assessment built from real security experience that gives IT departments, compliance teams, and security professionals a structured way to evaluate any AI tool before it touches sensitive data.
Why This Exists
Most organizations have vendor assessment processes for traditional software. Those processes were not built for AI.
The difference matters. When you onboard a standard SaaS tool, you’re evaluating access controls, uptime, and data handling. When you onboard an AI tool, you’re evaluating all of that — plus a set of risks that didn’t exist before:
- Is your data being used to train someone else’s model?
- Does the AI inside that tool depend on a third-party API you never agreed to share data with?
- If you’re in healthcare, has the vendor signed a BAA that specifically covers their AI components — not just their core product?
- What happens to your prompts and outputs after the session ends?
These are not hypothetical concerns. In 2023, Samsung engineers pasted proprietary source code into ChatGPT to debug a problem. That code became part of OpenAI’s training data. Samsung banned generative AI internally shortly after — but the data was already gone.
That incident happened because nobody asked the right questions before the tool was in use.
What’s in the Document
The questionnaire is organized into ten sections covering the full lifecycle of AI vendor risk. Here’s what each section addresses:
Section 1 — Data Handling & Privacy
The most important section and the one vendors are least prepared to answer. These questions expose what actually happens to your data once it leaves your environment — including whether the vendor even knows which AI components are processing it.
Key questions include whether the vendor maintains a bill of materials for all AI dependencies, whether customer data is used for model training, and whether a HIPAA-compliant Business Associate Agreement covers AI processing specifically.
Section 2 — Access Control & Authentication
Evaluates whether the vendor’s platform meets enterprise identity and access standards. Covers SSO enforcement, MFA policy, role-based access control for AI features, audit logging of AI interactions, and API key scoping.
Section 3 — Compliance Certifications
Covers SOC 2 Type II, ISO 27001, FedRAMP, GDPR Standard Contractual Clauses, Data Protection Impact Assessments, and EU AI Act risk classification. The critical point here: certifications only matter if their scope explicitly covers the AI features you are evaluating. A SOC 2 report written before a vendor added AI features tells you nothing about the AI risk.
Section 4 — Model & Output Risk
Addresses AI-specific risks that traditional security assessments miss entirely — data isolation in fine-tuned models, content guardrails, hallucination handling, prompt injection vulnerabilities, and intellectual property ownership of AI-generated outputs.
Section 5 — Vendor Security Posture
Evaluates the vendor’s fundamental security program: penetration testing frequency and scope, vulnerability disclosure programs, incident notification SLAs, cyber liability insurance, and uptime guarantees for AI features specifically.
Section 6 — Contract & Business Risk
The questions nobody asks until something goes wrong. Covers what happens to your data if the vendor is acquired, data export and portability guarantees, IP indemnification for AI-generated content, API rate limit terms, and change notification requirements.
Section 7 — AI-Specific Governance
Addresses the emerging regulatory landscape. Responsible AI policies, training data documentation and bias evaluation, third-party AI safety audits using MITRE ATLAS and OWASP LLM Top 10 frameworks, and data deletion processes that cover AI training data specifically.
Section 8 — Scoring Rubric
A structured scoring framework so assessments produce comparable, documented risk ratings rather than subjective impressions. Includes risk weighting by question severity, overall score interpretation, and automatic hold conditions.
Section 9 — Implementation Guide
Practical guidance on how to send, receive, and act on questionnaire responses — including escalation triggers, how to handle vague answers, and how to build annual reassessment into your vendor management calendar.
Section 10 — Glossary
18 key terms defined — from BAA and CAIQ to prompt injection, model extraction, and SBOM. Useful for teams who are new to AI security concepts.

How to Use It
Step 1 — Identify the vendor and data scope. Before sending the questionnaire, document what types of data the vendor will process. PHI, PII, financial records, legal work product — the data types determine which questions are CRITICAL versus informational.
Step 2 — Send with a deadline. Ten business days is the standard. Longer deadlines produce lower-quality responses. Include a request for supporting documentation upfront — ask for SOC 2 reports, BAAs, and DPAs at the same time as the questionnaire.
Step 3 — Score as you review. Each question includes a risk level and scoring guidance. Don’t wait until the full document is returned to start scoring — flag incomplete or vague responses immediately and schedule a follow-up call.
Step 4 — Apply the automatic hold conditions. Regardless of overall score, certain responses should trigger an immediate hold: a vendor who cannot identify all AI components in their product, a vendor who cannot provide a BAA for healthcare data, a vendor whose SOC 2 scope does not cover AI features.
Step 5 — Document and file. The completed questionnaire and score become part of your vendor record. For any vendor with access to sensitive data, build annual reassessment into your calendar.
A Note on Vendor Responses
The most important thing to understand about this questionnaire is that how a vendor responds tells you as much as what they respond.
A vendor who answers every question confidently, provides documentation without being asked, and proactively flags limitations in their compliance coverage is demonstrating security maturity. A vendor who gives vague answers, claims to be “SOC 2 compliant” without providing the report, or cannot tell you which AI APIs their product depends on is demonstrating the opposite.
Evasive answers to CRITICAL questions are not a negotiating position. They are a risk signal.
